If you wish to use your insurance, download the Insurance Verification Form in either pdf or Microsoft Word doc, complete it fully, and send to me via email. I will verify your coverage. If your plan provides coverage, I will process the claim for you, and you are responsible for the co-pay amount at the time the services are rendered. If I am not a provider with your insurance carrier, I can provide service as an out-of-network provider. In that case, you are responsible for the full fee at the time of service, and your insurance carrier will reimburse you directly for a percentage of the fees.
